현재 남성형 탈모 치료제 세계 판매 1위 달성
국내 식약처 승인 17년, 대한민국 남성의 삶의 질 개선 다짐
한국MSD (대표 아비 벤쇼산)는 지난 4월 17일 한국MSD 본사에서 남성형 탈모 치료제 프로페시아®의 FDA 승인 20주년을 맞아 “프로페시아®, 20주년의 자신감” 캠페인의 시작을 알리는 선포식을 진행했다.
“프로페시아®, 20주년의 자신감” 캠페인은 프로페시아®가 1997년 경구용 남성형 탈모 치료제로는 최초로 미국 식품의약국(FDA)의 승인을 받은 이후, 남성형 탈모에 새로운 치료 옵션을 제시하며 현재 남성형 탈모 치료제(minoxidil, finasteride) 중 세계 판매 1위 경구용 남성형 탈모 치료제로 성장한 지난 20년을 기념하고, 앞으로도 다양한 임상 연구를 통해 입증된 효과와 안전성 프로파일을 바탕으로 남성형 탈모 환자들의 삶의 질 개선에 기여할 것을 다짐하기 위해 기획되었다.
한국MSD는 “프로페시아®, 20주년의 자신감” 캠페인의 본격적인 시작에 앞서, 풍성한 모발을 가진 남성의 모습을 형상화한 프로페시아®의 심볼 마크 모라이언(毛 Lion)과 아라비아 숫자 20을 접목시켜 프로페시아®의 20주년 자신감을 상징하는 특별 로고를 제작했다. 또한 캠페인이 진행되는 2017년 한 해 동안 ▲프로페시아®의 의미 있는 임상데이터를 짚어보는 의료진 대상 심포지움 개최, ▲남성형 탈모 환자의 효과적인 치료를 돕기 위한 의료진 대상 디테일링(detailing) 자료 개발, ▲제품 개발부터 승인까지의 역사를 알아보는 프로페시아® 동영상 제작 등 프로페시아®의 FDA 승인 20주년을 기념하는 다양한 캠페인 활동을 펼칠 예정이다.
한국MSD의 경구용 남성형 탈모 치료제 프로페시아®는 1974년 도미니카공화국에서 발견된 선천적 제 2형 5-α 환원효소 결핍 모델에 대한 연구를 바탕으로 피나스테리드 약물 개발에 착수, 남성형 탈모 치료를 위한 적정 용량을 확립하였으며, 1997년 미국 식품의약국(FDA)의 승인을 거쳐 탄생하게 되었다.
5년 간 18-41세 나이의 경도-중증도 남성형 탈모 환자 1,553명 대상 무작위 배정, 이중맹검, 위약대조로 진행한 연구 결과, 전체적 사진 평가(global photographic assessment)에서 프로페시아®를 복용한 환자의 90% (vs. 위약 25%)에서 가시적인 탈모가 더 이상 진행되지 않았다. 또한, 108주 간 19-41세 나이의 건강한 남성 181명 대상 다기관, 무작위 배정, 이중맹검, 위약대조로 진행한 연구 결과, 프로페시아®는 정자수와 정자의 운동성에 위약과 비교해 유의한 영향을 미치지 않는 것으로 확인되었다.
한국MSD 프로페시아® 마케팅팀 이보아 과장은 "5년 간의 임상 연구를 통해 입증된 효과와 우수한 내약성은 프로페시아®가 남성형 탈모 치료의 시장을 이끌어 올 수 있었던 원동력”이라며, “특히 올해는 프로페시아®가 FDA 승인 20주년을 맞이하는 뜻 깊은 해인 만큼, 앞으로도 프로페시아®는 유일한 FDA 승인 경구용 남성형 탈모 치료제로 남성형 탈모 환자들의 행복한 삶을 위해 끊임없이 노력할 것”이라고 말했다.
한편, 한국MSD의 경구용 남성형 탈모 치료제 프로페시아®는 국내에서는 2000년에 허가를 받았으며, 경구용 남성형 탈모 치료제로는 유일하게 미국 식품의약국(FDA)과 국내 식품의약품안전처(MFDS)의 승인을 모두 받았다. 현재 미국, 영국, 중국 등 전 세계 54개국에서 판매되고 있으며, 남성형 탈모 치료제 (Minoxidil, Finasteride) 중 세계 매출 1위를 차지하고 있다. 또한 일본 및 유럽, 아시아 컨센서스 위원회 가이드라인에서 남성형 탈모 치료에 추천되는 치료제로 선정된 바 있다.

**Efficacy was demonstrated for all end points(Hair count, global photographic assessment, patient self-assessment, Investigator assessment) for finasteride at doses of 0.2 mg/day or higher, with 1 and 5 mg demonstrating similar efficacy that was superior to lower doses.7
*** [Primary endpoint result]: In the group that received finasteride for up to five years(Fin → Fin), there were significant increases in hair counts over five years (p < 0.001 versus baseline for all time points).8
a. The first clinical study (pilot study), conducted in 13centers in the United States, randomized 227 men into a12-month, double-blind, placebo-controlled study to evaluatethe effects of finasteride 5 mg/day on male patternhair loss. The second clinical trial (dose range study), conductedin 23 centers in the United States, randomized 466men into a 6-month, double-blind, placebo controlledstudy to evaluate the effects of low doses (1, 0.2, and 0.01mg/day) of finasteride. Men completing the 6-month doserange study were eligible to continue into a double-blindextension study without placebo for an additional 6months. In the 6-month dose range extension study,patients continued on the same therapy they received duringthe first 6 months or, if initially randomized to placebo, were rerandomized to one of the 3 doses of finasteride. 7
b. This study initiated two initial, 1-year, randomized, double-blind, placebo controlled studies and both were continued as four consecutive, 1-year, double-blind, placebo controlled extension studies. Men aged 18 to 41 years, with mild to moderately severe vertex MPHL according to a modified Norwood/Hamilton classification scale (Ⅱ vertex, Ⅲ vertex, Ⅳ
or Ⅴ) were enrolled. In 1 year initial studies, 1553 men with MPHL received Finasteride 1mg/day or placebo for one year and in four consecutive 1 year extension studies, 1215 men received either Finasteride 1mg or placebo. The efficacy was evaluated by hair counts, patients and investigator assessments, and panel review of clinical photographs.8
c. Double-blind, placebo controlled multicenter study 181 men 19 to 41 years were randomized to receive 1mg. finasteride or placebo for 48 weeks followed by a 60-week off-drug period. Of the 181 men 79 were included in subset for the collection and analysis of sequential semen samples. The objective is to study the potential effect and reversibility of effect of 1mg finasteride daily on spermatogenesis, semen production, the prostate and serum prostate specific antigen in young men.9
